But why did it fail on a young car without modifications?
A failed/failing DIC/IDM has been known to result in raw gasoline being sent to the converter because ignition never happened. Raw gas in a hot catalyst ignites and destroys the converter.
A bad DIC/IDM could easily cause all of your other symptoms. Unfortunately it is a $300+ part and no one wants to start wildly swapping parts in that price range. Recent DIC/IDM units usually generate specific CEL codes indicating which cylinder did not ignite, or a code for random misfire. For whatever reason yours did not. This could indicate an additional fault in your DIC/IDM and/or a problem with the engine software in your ECU.
However this part still seems like the most likely candidate, and 100K miles in ideal conditions is a common life expectancy for one of these so you are approaching its natural end of life anyway.
My guess is a failing DIC/IDM that will need to be replaced, and my recommendation is to have the ECU "reflashed" with the latest software too. If the P0420 code persists after the above work is completed and other problems eliminated, then you will need a new converter too.
