You say the pulleys were done; what about the tensioner? It sits right over the alternator and can make quite a rattling raucous at idle. Sometimes people replace the pulley on the tensioner but not the tensioner.
My other not-so-fortunate thought would be the timing chain. Usually these last to 200-250k miles without making noise, but over time they can stretch and make a rattling sound, particularly when the engine is cold. Oil starvation due to oil sludge can accelerate the problem.
