how much driving? The top nuts of my konis were 2 nuts you lock together rather than 1 lock nut. I have never had a pair of nuts come unlocked, but these see *a lot* of vibration... long story short, the nuts backed off, the assy came apart, and was banging around in the wheel well. Not easy to spot until it all fell apart.
Moral to the story: use locktite on those nuts as well as tightening thoroughly!
(that might not be your problem, but it's worth pulling the wheel off to have a look!)
