2001 9-5 Sedan suddenly started running rough, misfiring and would not run at idle speed, only if the rev's were kept above 3000. Made it home, just barely. I thought about the clues and codes (plus the car has been religiously maintained) and it seemed like it might be a vacuum leak. Went over the entire vacuum system with a strong flashlight looking for a leak. Finally noticed the vacuum connector on the intake manifold near the coolant bypass valve was loose. This is sealed with an o-ring, and held on by a bolt with a 10 mm head, which was quite loose. I removed the bolt and connector, lubricated the o-ring, reinstalled it in the manifold and replaced the bolt. The car runs fine now.
