The MT does not shift anymore. Disintegrated clutch?

Our 9-5 manual transmission cannot be shifted anymore with the engine running. When starting the car in the 1st or the reverse gear it immediately starts moving - even with the clutch depressed. The brake fluid level is fine. The clutch pedal pressure feels normal.
What could be causing this issue? The car has 276k miles currently and the clutch (basket + disk) as well as the throwout bearing were replaced less than 93k ago.
