Agree that this could be a case of thinking zebras when hearing hoof beats, but what I am seeing would appear to rule out the obvious possible causes.
The fuel filter and plugs were replaced at 120K. One of the first things I did was pull the plugs, re-gap, and reinstall using dielectric grease. I wouldn't think plugs/filter are factors because the engine bucking/stumbling only happens for a few minutes and just once per trip always beginning about 5-10 minutes from a cold start, and also because the engine will rev without hesitation if given gas while the stumble is occurring.
Wild speculation is one of the OBDII monitors that run only once per trip (e.g. O2, CAT) are doing an invasive test and there is something amiss (weak catalytic converter?) resulting in the stumble but not of sufficient magnitude to result in a code. The test eventually runs to completion and from that point on the engine runs fine.
