One (not very pleasant) possibility is that there is a bad head gasket allowing exhaust gas to leak into the cooling system. The gas winds up as a bubble in the heater core and prevents the coolant (or "heatant" in this case) from flowing thru the core and warming the air. A tech can check this by placing the sensor of an exhaust gas analyzer (usually used for emissions testing) over (not in!) the coolant in the overflow bottle. If it shows the presence of exhaust gases coming out of the coolant, you've got a problem.
