A call will go through to the enrollment side of OnStar. Since the system is not active they will not know where you are.
If you have service and the airbags go off or you push the emergency button, those calls go to a special section of the call center. The advisor has on their computer screen your info, a map with you location and the local phone number of the 911 center nearest you. These operators are specially trained and call the local 911 center summons help and end up with a 3 way call with OnStar you and the 911 center.
OnStar is a service. If you choose not to pay for the service that they provide, that's your choice. Don't expect them to offer a service for you for free.
