Maybe the gauge is out of calibration. I don't know if it's possible to re-calibrate the gague, but maybe you can. If not, a used VDO volt gauges are pretty cheap on ebay.
Maybe you're not getting a good source of voltage or maybe you have a lousy grounding point. It really doesn't matter where you're grounded as long as it's a good ground. Same is true for the power signal. I actually ran my power wire all the way to the battery. As a result, the gauge is always-on but that's not much of a power draw.
The other option is that the gauge is reading correctly and you've always been running at less than 12v. That's probably unlikely if you haven't had a lot of electrical/charging issues.
