I suspect the radiator is partially clogged. Not enough to cause a problem at lower cooling water flow rates at idle or for the most part around town...most of the time. You might be able to still have it flushed to remove the deposits at this point. The fan being on or off while on the highway is meaningless, there is substantial air flow through the radiator without the need for fans. If it had been the other way around, cool on the highway/warm at idle/stop and go, then one looks at faulty fan operation. If it is hot during idle or on highway..then stuck open thermosat or clogged radiator.
