My experience has been that it's caused by a failure of the switch. My bet is that you might have had an aftermarket switch installed.
One of the aftermarket switches I bought (some 6 years years ago) failed open - the contacts fell apart after about a year, so the brake lights didn't activate. Lucky that the ol' SPG wasn't rear-ended on the interstate.
I got an official brake light switch from the dealer, installed it, and haven't had a problem. Sad to say, the aftermarket switches are pretty poor.
You'll find that, after the first install with the lower dash off, you will be able to feel around and install the next one without removing the lower dash.
