If it worked fine with no noise before the loss of 5th and you are now getting 3rd instead of 5th I would think it is only linkage issue. In my experience, there are two weak points in the linkage (which is actually a very straight forward system): the rubber shift coupler (as justin mentioned) which can disintegrate . . . or the rubber bushing inside and toward the front of the shift console inside the car. The former item is easy to inspect visually if you can get under the car. The later requires a bit more disassembly of the interior to see it. The shift bushing can break down to the point where it is so sloppy that it does not allow the sideways action of the shift level (as you move it toward fifth) to properly be translated into rotational movement in the shift shaft going to the transmission.
To get a good look at the shift bushing you will need to remove the knee bolster, center console, front carpet set (or alternatively cut a slit in the carpet forward of the shift lever), and black ventilation ducting. Once you have a clear view of the front of the shift console (with the shaft emerging from the front) shift the car through the gears. There should be minimal side-side movement of the shaft (where it emerges from the console) . . . you should just be seeing a precise rotation of the shaft as you move the gear lever from side to side.
