I can't answer all of your questions, but I just came in from repairing my short shifter that had a broken reverse lock out pin. I have been taking various shifters apart for the past couple of weeks trying to figure out how to fix the broken reverse lock out pin. I got several junk yard shifters and after taking them apart and putting back together I was able to repair my short shifter.
To your questions:
2. I used to use some thick grease, like axle grease, but now use white lithium grease.
4. 1/4" should be enough. The bottom pin should be protruding about 7/8" below the bottom of the plastic ball at the rest position. I pulled the collar up and the pin went up about 1/4". You can adjust the height of the pin by removing the clip that hold the plastic ball. Slide the ball off the shifter shaft, then you can spin the lock out pin from the bottom. Clockwise will screw the pin in, counter clockwise will move the pin out.
