1st of all I do appreciate the way you get your question asked since those 47 secs are worth a thousand of posts. Although a small video clip could be even of bit more help so we're able to see the rattle and what a particular point was used to record a certain noise from.

Having a car stationary and engine running there are the following parts moving:
1: pistons
2: con-rods
3: crankshaft
4: timing chain
5: camshafts
6: intake and exhaust valves
7: ignition distributor
8: water pump
9: alternator
10: AC compressor (if one is installed)
11: oil pump
12: clutch and clutch shaft
13: transmission chain
14: input shaft
15: intermediate shaft aka layshaft
Parts 12-15 should stop moving in a few seconds right after you step on the clutch pedal.

You said having clutch disengaged or released makes no difference so it should not be smth about transmission. As for your note regarding the IV and V gear I think your just stop hearing the rattle but this doesn't mean it goes away. I could advise you to do engine braking on the V gear and I bet you should be able to hear this rattle but I won't do that since it will probably kill your engine.

Assumption #1 (most probably): timing chain and-or the tensioner.
You'd start with going under the valve cover. It's not a big deal and probably you'll need only spare valve-cover seals. You'll be able to inspect the timing chain and the tensioner and all the stuff around them. Also you'll be able to check the gear about valves and camshafts.

Assumption #2 (of much less probability): smth in the block.
Try to isolate the cylinder that this rattle is referred to by unplugging connector from corresponding injector valve. If you go with spark plug wires then you can kill your ignition coil and-or dizzy cap and rotor.

Assumption #3 (about the same probability as #2 but much easier to troubleshoot): belts.
Just remove all the belts and start the engine and let it idle for 10-30 seconds.

Hint: you can use a not too big hummer as a good stethoscope - just press its helve to the lobe of the ear and you'll hear all the noises from the unit you have your hammer touched with.

I would not drive this car a single yard nor I would start its engine without serious necessity until this issue is fixed.
