Just a thought, but check to see that the rack is secure in its mounts. You can either support it safely with jack stands, crawl underneath and check the big torx bolts or have a help turn the ignition to "on" but not start the car. Look down at the rack while the person inside turns the wheel. The rack body should not move.
Unfortunately, this is one of those times where most parts could be suspect.
