The belt reel goes behind the trim panel for the rear seat. In a car with "regular" seat belts, the belt passes through a hole in this panel, up to the B-pillar mount/pivot, then down to the floor, on the inboard side of the sill, just forward of the B pillar. (unless you got 91- belts, in which case, the third point mounts on the seat, where the lap bet would on the passive belt)
The main body of the belt is to the outboard side (if the reel assembly looks like a lower-case 'b', the straight part - where the belt tails off - goes closest to the panel). This is important so the reel locks correctly.
Holes for all the brackets should already be in the chassis. You'll need the different B pillar trim, as well.
