I will assume that you had turned the ignition to "run" in order to slip the car out of gear for the push and then back to "off" while attaching cables for jump-start. If so, that doesn't leave much opportunity for a push-related mishap. I suggest that you stick with your first thought of a poorly-timed failure of lock cylinder.
It's been a number of years since I had to replace mine (thank goodness!) but IIRC, there is a little paw that extends from the bottom of the cylinder and moves in a circular arc when you insert and turn the key. The resistance you ordinarily feel results from the paw moving the actual ignition switch between stations. If the paw broke off or otherwise disengaged from the switch you'd have what you are experiencing, so that's my guess.
Now, re: repair/replace options: I think adding a second switch is just a Bad Idea. Doable, perhaps, but not as simple and neat as fixing it right. The Bentley manual has a relatively clear description of how to do it. If you don't have one, you should get it, it's indispensible help for maintaining these cars. Also if you order the manual from a site sponsor (Eeuro sells them) buy a genuine replacement lock cylinder; not sure about the fit/suitability of generic replacements.
