From a dead stop, I had to start it in gear. 2nd.
Once it got going, I used the old speed-shifter's trick and used engine pitch and car speed, along with some ginger feathering of the gas pedal, to ease the shifter into the right gear.
Poor car ... at 214k miles and the harsh detriments of Syracuse's winter salt, it earned its keep.
