I think it is unlikely the flywheel is to blame. A glazed flywheel would be no-go from first drive, it wouldn't go from a-ok to slipping after a period of time. IMHO it's more likely they replaced the disc and not the pressure plate. I have seen that before - a bad pressure plate causes slipping, and an inattentive shop replaces the disc thinking that if the problem. Then, a little while later it wrecks the new disc due to insufficient clamping and it's back to square one.
Only way to know anything at this point is to take it apart and see what's broken...
