Like jimmy says, I would check the fans to see that they are good, and I would also confirm the fan switch is working (can short out to see if the fan runs). That said, if your rad is original, it is probably due to be replaced even if it is not completely gone yet... those kinds of symptoms can happen even with a working fan if the rad is bad enough (been there - sometimes the open road helps somewhat but not really back to normal). Cheap and easy replacement. Personally, at the same time as the rad swap, I would go for the 82 degree fan switch and 82 degree t-stat (and merc coolant) as per townsend. I have no issue running that combo even in our northern climate. Only issue is if you only change the tstat to 82, you'll get a temperature overshoot when you come off the highway... Oh, and I would also check the condition of all cooling system hoses to replace at the same time as the rad...
