That is a pretty unorthodox method with the steel angles and I was thinking (after seeing the pictures) that you might not have the pressure plate retracted sufficiently to get the assembly out. I almost always succeed with my #4 copper ring or some such BUT on occasion use the nut and bolt method I described above (AKA 3 fat nuts). That method is basically foolproof and will always work, it's just a little bit more work than using the slave to depress the pressure plate. Lifting the whole mess out requires that the slave and throw out bearing sink pretty far into the clutch cover assembly so you want to err on the side of too much compression not too little.
