Take a straightedge and make sure the plastic manifold is flat as a pancake. Put it on a glass or surface with a piece of wet/dry fine paper, and swirl it around to see if the edges and the bifurcation are on the same plane. I sanded on one and found so many high spots, it was surely going to leak. The normal reaction would be to keep tightening the screws upon noticing some seepage, and then the screws would undoubtedly collapse the plastic, leading to yet a larger leak path.....
Having new O rings seated well and with some spray adhesive to hold them in place before attaching to the metal interface would be helpful in particular if you're doing it insitu.
