It is the blue item under the middle of the intake manifold. Pull the connector off and check the resistance cold and again warmed up.
Here is the chart:
5800ohms at 32 degrees
3800ohms at 50 degrees
3000ohms at 58 degrees
2600ohms at 68 degrees
2000ohms at 76 degrees
1700ohms at 86 degrees
320ohms at 176 degrees
If yours is not in this range then you should replace it. Since you say the engine starts great when cold but your mileage is bad, the temp sensor circuit could either be open or staying at a high resistance when warm causing the engine to run rich. Or, the wiring is faulty and it is remaining open all the time. Try unplugging it and see if there is any change. If not, there is your problem...
