Catastrophic Bearing Failure

Hello All,
Last week my mechanic was driving down the highway at about 100kph when the car suddenly felt like it popped out of gear. He coasted to a stop and tried all the gears (no problem). As he let out the clutch in either 1st or reverse, the car didn't move and would start to stall.
Now that the car is back to the shop and he has had a look, it appears that the output shaft main bearing failed suddenly and the transmission stopped as the car was moving. Both axle shafts sheared and the bearing was partially crushed in the transmission.
Can anyone tell me if this is a common failure? There was absolutely ZERO noise or vibration prior to. I'm also wondering what else should be examined for damage.
