Rear clunking could be several things. Pull rear wheels and check that e-brake cables are not stuck. Also check rear pads for looseness, although they usually rattle rather than clunk. While you have the wheels off check the bushings on the rear control arms and the bushings on the transverse panhard rod that "locates" the rear axle. Bad bushings can cause clunks.
Transmissoin whine in 5th could be the beginning of the end for the trans. you might replace the trans oil with redline mtl and nurse more miles out of it.
Reverse clunking is caused by the clutch not fully disengaging or the pilot bearing in the flywheel dragging. So that you don't chip off the reverse gear, put the trans in second or third before quickly shifting to reverse. To really fix the problem you must address the lack of disengagement issue. If you have alot of free play before the clutch starts to disengage and the clutch does not release until you get to the floor, check clutch pedal for elongated hole where it connects to the clutch master cylinder. It's under the dash near the firewall. If bad, you can drill out the hole and put in a bigger pin, or get a new pedal. If it's OK, you can bleed the clutch hydraulics to remove any air, but I'll bet the problem is a bad pilot bearing or corrosion on the clutch shaft. To fix those, you have to pull the clutch. If the clutch is original, you might replace the clutch now and put a new pilot bearing in while it's out
