I keep getting back to possible mechanical sources since you seem to have covered most of the obvious electrical issues except for loose grounds. Last night I was thinking about the misfire and backfire symptoms and thought of timing. All of the phenomena that you are experiencing could be timing related. Is it possible that there is a problem with the timing chain/sprockets or with a loose crank position sensor that would cause a problem like this? The intermittant nature is still troubling since if the chain has slipped a tooth you would have a constant problem with misfiring. However, if it is very loose (say if the tensioner spring is broken), it could well be load dependent.
Just more random musings, but ones I would consider in your position...
