Does the fan change speed, or does it run full speed no matter what?
If the fan is running full speed no matter what, things that come to mind (in no particular order):
1- bad transistor; rarely new parts show up failed.
2- bad transistor - overheated it soldering it in place
3- The heat sink has shorted to chassis
4 - Transistor put in backwards
I don't mean to impune your soldering/electronics skills. Just some suggestions
