I wrote a post about this a few days ago... The nut on the end of the axle seats onto the wheel bearing and threads onto the axle... The bearing thus gets squeezed between the nut and the CV joint. That's what creates the pre-load on the bearing. As such, I can't see how you could lose the wheel unless that nut comes off. If your bearing is wearing out it is likely that it will start making a noise ugly enough to keep you from driving long before any parts of it actually fail. Even if it did actually disintegrate, I don't see how the wheel could come off. Now, the wheel could stop turning or get pointing in a strange direction, I suppose...
Do look carefully at ball joint and tie rod ends because if they fail it could be disastrous.
Usually a bad wheel bearing make a rumbling sound which is particularly noticeable as you roll to a stop. It will also exhibit some play. The noise will get worse with load; if your right bearing is bad, it will get louder on a left turn. By the time it becomes urgent, it will sound really bad. If it's clicking you're hearing, I'd keep looking. Is it a click or a clunk? I have a click that usually occurs only once when I first get in the car. I have no idea what it could be yet.
