Bad (or wrong) plugs and (some say) a lack of dielectric grease on the boots when reinstalling.
Bad (or wrong) plugs would also cause other problems, like the rough starts that you mentioned. These could also lead to DI failure. So how are the plugs on that car?
I don't think that something wrong in the wiring would have a direct impact on the DI, unless the harness to the DI got fried or something. Hopefully someone that knows more than I will chime in here.
