With the car off and cold, squeeze the hose. It will flex some, but if it feels softer or gummy there, replace it. If you see small cracks in the surface, replace it.
Quite frankly, If you suspect a hose, replace it. Hoses rot from the inside out. It's a tough call to say that a little swelling is OK. If you're trying to limp the car another month or two, wrap the hose with some duct tape. If you intend on keeping the car, go with your instincts and replace the hose. A busted upper radiator hose is no fun. When you go to replace it, I recommend that you use a rajor to carefully cut the hose at the radiator nipple. The nipple can be fragile.
