Carfax only has accidents that are reported to it. And not everybody reports accidents to their insurance company. So there are two places in the chain where information can be lost.
If somebody nailed his car on a guardrail one night, and didn't want to report it to his insurance because his rates would go up (or get his policy cancelled), he/she may decide to pay the repairs out-of-pocket, especially if he had a friend in the business who would do it cheap. And since most of the bolts were missing, cheap is the operative word.
