Seen it happen a couple of times in Sweden and mostly because the coolant mixture was not perfectly 50/50 so when the temp hit down in the neg 20s and 30s, the freeze plugs popped out.
Freeze plugs are also used as sacrificial anodes (like in sail boats) to attract corrosion away from other parts of the engine. That's why they rust through sometimes, again due to neglected antifreeze mixture or age of the coolant.
