NickF is correct, check the linkage. But the thing you want to look at specifically on the linkage, is the flexible shift coupling. The coupling is in the engine compartment between the firewall and the transmission. The coupling is a piece of rubber with two plates bonded to it. Over time the plate slowly separates from the rubber and the symptoms you describe are consistent with that failure. Have some one move the shifter while you shine a flashlight on the coupling. Check the coupling to see if it separates when the shift rod rotates.
