The temperature guage reads the engine temperature. The fan comes on based on the radiator temperature. It's the thermostat in between. If the thermostat is more than about 3 years old, replace it. It's a moving part, and it can stick. The engine would get hot, but the radiator would be nice and cool so the fan would stay off.
Also - since the gauge and the radiator temperature are loosely connected, it's not uncommon for the temp gauge to come pretty close to the red before the fan kicks on. You see a lot of temp swings on the older cars.
This isn't a problem on the newer 9000. Not because of any change to the cooling system - the temp gauge is not directly connected to the temperature sensor - the signal is conditioned by circuitry in the EDU. In later years, Saab changed the circuitry to smooth out the temp gauge swings, and make the gauge read lower. The car still gets as hot, but the owner doesn't worry. And it's actually just fine - if the car does get too hot, the gauge will swing up into the red. You just don't see all the intricate workings of the thermostat/radiator/engine load.
