This can be caused by either upper or lower motor mounts heading south, the tranny mount failing, the shift coupler or, as you note, some part of the clutch. I'm not sure the car being warmed up matters. My shifter was getting worse and worse and usually seemed better once the car was warmed up, but it was really the coupler that was shot.
Start simple. The coupler could be wearing out. This can be replaced or, as my indy has done on my '96, the tapered pin can be driven in more. He just did a new coupler and the car shifts beautifully.
Look at your upper mounts. How do they look? Then the lower mounts. If one or both are leaking then replace them.
The tranny mount is probably fine, but worth a look. Not hard to replace, but requires some planning and a way to support the tranny.
As for the clutch, how much travel is there before it engages/disengages? Is the brake/clutch fluid level OK?
