I had the exact same issue with my '98. Both rear rotors looked just like that.
Do you grease the sliding pins? Are they worn? Do you clean all parts of the caliper where the pads slide back and forth on when being applied and released?
I used a wire brush to clean all the mating surfaces of the caliper. I then used some anti-seize lube *lightly* on those surfaces so that the pads would move more easily on them. I also switched from using anti-seize lube on the sliding pins to brake grease, made for that job.
So far, I haven't had the problem since, and it's been 20K miles. I also flushed and refilled the brake fluid in the lines (don't know if that helped, but it was time).
