The belt should be riding in the center of the tensioner pulley. None of the belt should be not riding on the pulley. To amplify on yaofeng's comment, any misalignment can cause this. I have seen a broken mounting bolt on the tensioner assembly cause it to 'lean away' from the engine and cause a misalignment. I've also seen a bad main pulley (aka Harmonic Balancer) cause this- as the outer portion of the main pulley starts to move (either towards or away from the engine) because the rubber sandwich has failed, it directly misaligns the belt on the tensioner.
I'd immediately check the main pulley, and visually check the tensioner mount and idler pulley.
To add to T. Christopher's note - with the engine running, watch the tensioner pulley. It should jiggle a little, but no more than about 1/8 inch or so. Wait for the AC to kick in. The pulley should move to a new position quickly and smoothly, and resume its little jiggle there. There should be no overshoot by the pulley - go past the point, come back, and eventually settle down. Excess motion and overshoot are a sign the shock is shot.
