Plan:
1) Fill a drinking glass with six ounces of water and four squirts of dishwashing liquid. Take a paint brush and paint every intake delivery pipe joint, throttle body hose, throttle body/plenum joint and vacuum lines with this liquid. Start the engine, and learn which parts are blowing bubbles.
Failing to see any bubbles, paint all the joints and hoses a second time.
If you still see no bubbles, proceed to step two.
2) Your case is the reason OBDII was developed. The cars since 96 can return engine load, throttle position, and manifold pressure in one scan.
You don't have that option, so take a cab to the parts store and buy a map sensor and some 5/32" vacuum line. Throw away the 18 year old map sensor and the associated vacuum line, and replace with new parts.
If the sensor doesn't improve things, you must have a severe air leak. Proceed to step three.
3) Heavy lifting. The intake manifold gasket must be the issue, and the fix is to replace it.
