I knew they were also centered by the hub and interior of center of wheel, but that can also be pounded out if lugs are loose and wheel gets run a ways on them. So I didn't mention checking that.
The cone shaped seats do absolutely play a role. Check the lug bolt seats in the wheels.
The sign it could well be the wheels is it happens with his summer wheels but not the winter ones.
I once was narrowly missed by a bounding wheel and tire, that came off a 4x4 pickup speeding towards me from the other direction. I helped chase it down and got it back to the truck. The woman driving said it had been happening and her husband keeps tightening it up, didn't understand why it kept coming loose.
The lug holes were all pounded out, out of round, no conical shoulders left on them.
I told her to make him get a new wheel and trash the old one. A front wheel. No one hurt and truck amazingly came to a stop without damage.
