1. There is a four-wire connector that plugs into the ACC fan speed control module; on my car one of the connector pins was very oxidized and was replaced when doing the motor. I imagine that if it had gotten a bit worse, the motor would not have run.
2. The fan speed is controlled by a large transistor in the fan speed control module. Typical failure mode is that the fan runs at full speed and can't be turned down, but if the failure is an open vs. a short then the fan wouldn't run.
