I would suspect the head first.
I took a '78 99 engine apart with about 225,000 miles on it, and I could have easily run it for another 100,000+ miles. All that engine really needed was a valve job. There was no ridge at the top of the cylinder, which I almost always find on a high mileage V-4. None of the 99/900 engines I've had apart had any ridge. The metalurgy of cast iron in the '72 and later SAAB-built engines is incredibly hard. I had an engine bored (another long story) and my machine shop said that meatalurgically it was one of the hardest blocks he'd ever run across. They just don't wear. On the other hand, my ex-wife had a '82, pre-APC turbo 900 that had a piston ring break and gouge a big slot in the wall of one cylinder, so that can happen if you have detonation.
