Could you have adjusted the release arm in so far in the bolt heads on the clutch cover are hitting the release-bearing fork as they spin?
Have you got 4mm free play where the slave cylinder pushrod meets the release arm?
When you put in the new release bearing, did you check the mounting holes in the fork to make sure they aren't elongated through wear?
