The engine oil cooler sits between the two cylinder heads submerged into the engine block.
The oil cooler is known to crack and mix coolant/oil, so that is the first place to look when encountering a problem like this on the V6 engines.
When the cyl heads were off they should have removed the oil cooler and pressure tested it since it's really easy to do at that time.
The oil cooler is expensive though (close to $500.00)
All the cooling hoses should be replace too since they are contaminated by oil and most likely have started to disolve from the petroleum based product.
