water pump pulley eat a hole Inside or Outside?

hmmm....if the water pump pulley was tilting inward... would it dig a hole on the *inside* of the pump assembly.. or would it "rub" a hole on the edge of the *exterior* of the engine?
mechanic says it's the head gasket.. but maybe he's guessing? what other things should I ask him to check for?
