Had this happen on my 9-5 2.3lpt. The first fix was to remove the exhaust manifold and install a new gasket. The same manifold bolt loosened up again. The initial sign of a problem is a brown color forming where you now have a black deposit. Mine also had valve clatter when cold as if it needed a valve adjustment. The reason the initial fix did not work was because the manifold had warped and needed to be replaced. Good luck,
