A/C hissing, blows warm, diagnostic ideas?

Hi all,
'99 9-3 SE (LPT)
My A/C continually makes a hissing noise and now doesn't get cold. I had Midas check the system last year and they couldn't find a leak. They said they topped it off. It was better, but not really blowing as cold as it should all the time. This summer, same problem but it got increasingly worse. Now it hisses all the time and blows warm. When I turn off the A/C, the hissing stops.
Thanks to Steve L, I bought a can of R134 with a gauge. I got a high reading (65) from what I believe is the high-pressure valve (black cap, near the radiator on the driver's side). The gauge does not fit on what I believe is the low pressure valve (green cap, near the back of the engine compartment on the passenger side.
While I was fishing around for the valve, I found a small flexible rubber hose that seems to run toward the driver's side headlight. It is disconnected on the other end, near the battery. I couldn't find a connection for it near the battery, although it clearly should connect somewhere. The hose is the same type used to carry washer fluid.
Thanks in advance for your help with the following questions:
1) Am I measuring the correct pressure valve? If not, do I need a new fitting?
2) Any other thoughts on the cause of the A/C issue? What should I check?
3) Any thoughts on the mystery hose?
Thanks in advance for any advice.
