Stumbling on initial take-off... why?

2000 Viggen, 75k miles:
When I start the Viggen in the morning, back out of driveway, shift to 1st and take off... it frequently "stumbles". It's gotten to where I now feel I need to really feather the throttle and gradually engage the clutch. Frustrating. Often as I'm just beging to roll away, suddenly the revs will drop to nothing and catch again... causes the car to slow and then WHAM it takes off with a jarring jerk. Hard on drivetrain I'm sure.
What might cause this? A few other things I can say about it... it's only that initial take off. I stop at an intersection a minute later and leave it smoothly. Also, the temps here have dropped... 35-40F in the morning now. Also, the car does seem a bit reluctant to start... cranks a few extra times. And occasionally the engine will simply quit after I've backed out the driveway and clutched to shift. My thoughts -- could this be throttle positions sensor, or MAF? How about the K&N filter? (No other mods the car.)
