#2 - the pump runs forward for the windshield and headlights, backwards for the rear window. It could be a plugged line, but it's more likely that the washer pump is half-dead. IIRC the pump is integrated into the reservoir, but it's ~$100 and half an hour to replace, again, IIRC.
#3 - if you're lucky it'll be a connection, but it's probably one of the heater pads. Not cheap, but DIY-able.
