The reason to do it is to prevent that part of the system from failing and screwing up the timing -
I had 150k and rattles:
My plastic guides were pretty well worn out, and this was a CLEAN engine w/ regular oil changes - those plastic guides JUST DON'T LAST 200k like the old metal ones did!!!
You can waste the $$$ replacing it all (I had to replace my timing chain tensioner guide anyway, the end wore thin, so I did it myself, but there's no reason to spend the $ on the balance set)
My balance shaft sprockets were PERFECT, no wear, but it rattled anyway due to the guides!
Open it up and let us know what you find - I'd cut it, then remove and replace the tensioner arm like I did - it takes the MOST wear since all of the tension from the chain is on the end of that arms, and it's obvious what metal on plastic over 150k does... Then you can likely go another 150k before one of the other plastic guides is near failing.
