Rob, assuming that this car doesn't have the ACC, the path for the rear defroster element is as follows:
1. Battery to the switch pin #4 comes from 10A fuse #21.
2. When the switch is operated, battery passes out pin #5 to pin #66 of the ICE unit.
3. Pin #35 of the ICE sends a ground to pin #85 of the rear window relay.
4. This operates the relay and battery from 30A fuse #1 gets cut through the relay and exits on pin #2.
5. Pin #2 provides battery to the rear wind heater as well as the heated mirrors.
6. Battery on pin #85 also causes the lamp to light in the window switch.
So, lots of things need to happen just to get the lamp to light. I would start checking the pins I mentioned with a VOM to see what's on each pin.
The following hardware could be faulty: fuses 1 & 21, heater switch, heater relay, ICE module, heater element on rear window (but not probable, wiring to/from the pins I mentioned.
